"In the Flesh?" is the opening track, and introduces the story concept of the album. "In the Flesh" is the twenty-first song of the album, and is a reprise of the first with a choir, different verses and more extended instrumentation. The title is a reference to the band's 1977 In the Flesh Tour, during which Roger Waters, in frustration, spat at a fan attempting to climb the fence separating the band from the crowd. (en)

Il titolo, traducibile in italiano in senso figurato con "di persona?", oppure "in carne e ossa", deriva dall'In the Flesh Tour, serie di concerti che la band fece nel 1977, durante il quale Roger Waters, in un attacco d'ira, sputò a un fan che aveva scagliato una bottiglia di birra verso di lui; Waters poi dichiarò che da questo episodio scaturì la sua riflessione sull'incomunicabilità fra artista e pubblicò, che gli avrebbe fornito l'ispirazione per il tema del successivo album, The Wall.
