Cairns studierte Medizin an der Universität Oxford (Bachelor 1943, Promotion 1952) und arbeitete dann als Virologe am Walter and Eliza Hall Institute in Melbourne, am Virus Research Institute in Entebbe und an der John Curtin School of Medical Research der Australian National University. 1960/61 war er Gastwissenschaftler am Cold Spring Harbor Laboratory, dessen Direktor er 1963 bis 1968 war.
