Der Fluss entsteht durch den Zusammenfluss seiner beiden Quellflüsse North Llano River und South Llano River nahe der Ortschaft Junction und mündet nahe der Siedlung in den Lake Lyndon B. Johnson, einen Stausee des Colorado. Der Fluss fließt auf seinem Weg durch die texanischen Countys Kimble, Mason und Llano in ostnordöstlicher Richtung, bis er kurz hinter der Stadt Llano für die letzten etwa 30 km vor der Mündung nach Südosten abbiegt. It drains part of the Edwards Plateau in Texas Hill Country northwest of Austin. This river is one of the few popular fly fishing destinations in the state of Texas, and contains Guadalupe bass, the state fish of Texas.
