| - TIPS (auch: TIPSS) ist die Abkürzung für einen transjugulären intrahepatischen portosystemischen (Stent-)Shunt und bezeichnet eine angiografisch geschaffene Verbindung zwischen der Pfortader und der Lebervene durch die Leber hindurch (portosystemischer Shunt). Mit dem TIPS soll erreicht werden, dass ein gewisser Teil des Blutflusses von der Pfortader nicht in die Leber, sondern direkt in den großen Blutkreislauf fließt. Eingesetzt wird ein TIPS in der Behandlung eines Pfortaderhochdrucks. (de)
- La derivación portosistémica intrahepática transyugular (DPIT) un procedimiento establecido para tratar las complicaciones derivadas de la hipertensión portal, que incluyen: sangrado por várices esofágicas, ascitis refractaria, hidrotórax hepático, síndrome hepatorrenal y hepatopulmonar. Últimamente también se ha implementado su uso en el síndrome de Budd- Chiari y en la enfermedad venooclusiva. Este procedimiento es llevado a cabo en una sala de cirugía hemodinámica, lo realiza un radiólogo intervencionista, bajo sedación consciente, en ocasiones excepcionales es necesaria la intubación endotraqueal y con técnica aséptica quirúrgica, igualmente se da profilaxis antibiótica para disminuir el riesgo de infección. É usado no tratamento de hipertensão portal, que geralmente resulta de cirrose hepática, a qual causa hemorragias intestinais, hemorragias esofágicas e acumulação de líquido no abdómen. - Transjugulární intrahepatální portosystémová spojka (TIPS) je metodou intervenční radiologie sloužící ke snížení portální hypertenze zejména u pacientů s dlouhodobě nekontrolovatelným krvácením z nebo s refrakterním ascitem. Spočívá ve vytvoření spojky, kanálu, zkratu mezi portálním řečištěm (povodí cév před játry) a systémovým řečištěm (dolní dutou žilou). Provádí se miniinvazivně, tzn. jen pomocí nástrojů zavedených do žil. (transjugulárně). Tato symptomatická léčba není metodou první volby zejména pro riziko výskytu či zhoršení jaterní encefalopatie, jiné mnohé komplikace a v neposlední řadě vysokou cenu. Zakladatelem TIPS je Josef Raš, český lékař, který v exilu poprvé vyslovil myšlenku tohoto teraputického postupu. Prvního klinického užití se tato metoda dočkala v roce 1988 (G.Richte (cs)

It is used to treat portal hypertension (which is often due to liver cirrhosis) which frequently leads to intestinal bleeding, life-threatening esophageal bleeding (esophageal varices) and the buildup of fluid within the abdomen (ascites). An interventional radiologist creates the shunt using an image-guided endovascular (via the blood vessels) approach, with the jugular vein as the usual entry site. (en)
