An intrastromal corneal ring segment (ICRS) (also known as intrastromal corneal ring, corneal implant or corneal insert) is a small device implanted in the eye to correct vision. The procedure involves an ophthalmologist who makes a small incision in the cornea of the eye and inserts two crescent or semi-circular shaped ring segments between the layers of the corneal stroma, one on each side of the pupil. The embedding of the two rings in the cornea is intended to flatten the cornea and change the refraction of light passing through the cornea on its way into the eye.

Eine Hornhautverkrümmung sollte nicht vorliegen. Die Operation sollte nur von einem erfahrenen Hornhautchirurgen vorgenommen werden. Das Risiko eines Schnittfehlers kann bei Einsatz des Femtosekundenlasers reduziert werden. Im Fall einer Über- oder Unterkorrektur können die Ringsegmente ausgetauscht oder auch wieder entfernt werden. Obwohl die Ergebnisse sehr zufriedenstellend und stabil waren, hat sich dieses Operationsverfahren bisher nicht durchgesetzt, da zur gleichen Zeit ein chirurgisch einfacher durchzuführendes Verfahren, die Laserkorrektur mittels Excimerlaser, entstand. Ein Vorteil der ICRS besteht jedoch in der Reversibilität des Verfahrens. Derzeit werden die ICRS vor allem zur Stabilisierung der Hornhaut bei Keratokonus eingesetzt. (de)

El cirujano los coloca en forma de anillo en el interior de la córnea, en su zona periférica, aislados del exterior, lejos de la zona central del ojo y guiados por un canal predisecado con un disector específico que tuneliza. La intervención se realiza en un ojo por vez y en caso de ser necesario en ambos. Se espera aproximadamente un intervalo de 30 días entre la cirugía de un ojo y la del otro. Entre las intervenciones más frecuentes se encuentran pacientes afectados por queratocono, , ectasia corneal e irregularidades corneales que se presentan tras haberse sometido a una cirugía láser.
